tui: add preview margin to menu scrolling

The footer menu now starts scrolling before the selection reaches
the viewport edge, keeping surrounding items visible for context.
Extract offset logic into pure helpers and add tests.
Simplify theme primary color selection to use fixed ansi colors
instead of a contrast/chroma heuristic, and suppress diff
backgrounds in scrollback mode for cleaner rendering.

cli: add variant selection panel to command menu
...
Previously model variants (reasoning effort) could only be cycled
with a keybind. Add a searchable selection panel accessible from the
command palette, matching the existing model selection flow. The
footer hint bar now advertises the command palette keybind instead
of the variant cycle shortcut.

run: clear prompt draft on Ctrl-c before exit
...
When the prompt has text, Ctrl-c now clears it instead of
immediately starting the two-press exit sequence. An empty prompt
still triggers exit as before. The clear key is configurable via
the input_clear keybind.
Model labels now show provider display names (e.g. "GPT-5 · OpenAI")
instead of raw IDs, preferring configured providers over the full
provider list for resolution.
